This clinic is a primary medical home to many pediatric patients and their families in Baltimore City and the surrounding areas. Frequently, these families are of low income and public medical insurance, therefore the multidisciplinary team approach to these episodic and well child appointments offer a distinct advantage to these families that often suffer from issues with referrals, follow up appointments, transportation, and monetary resources. Throughout well child or episodic appointments, the practitioner highlights aspects of the patient’s life that carry heightened risk for negative health outcomes and provides education as well as initiates appropriate referrals for further aid. Within the Harriet Lane Clinic, in house referrals can be ordered and completed the same day, thus providing a unique opportunity for these patients and families. Typical referrals include risk reduction, social work, mental health, adolescent health, nutrition, breastfeeding and lactation consultants, as well as behavior and developmental specialists. The practitioner provides care to the patient age 0-21 years old according to current recommendations by the AAP and CDC, specifically in regards to screening for lead, anemia, dyslipidemia, STI’s, vision and hearing deficits. Immunizations are provided with adherence to the CDC recommendation schedule and individual patient factors. Show less

Patients are scheduled for 120 minute appointments for initial visit due to the complex process of collecting a detailed personal history, developing at minimum a three generational family pedigree, and completing a dysmorphology specific assessment. The dysmorphology assessment includes a full physical and neurologic assessment, as well as measurements and ratios of different body parts. Genetic syndromes that are commonly referred include Congenital Heart Defects, Ehlers Danlos Syndrome, Noonan Syndrome, Stickler Syndrome, Tumor Syndromes or Syndromes that carry heightened risk of Cancer, and Chromosomal Aneuploidy, Microdeletions, and Single Gene Mutations. A thorough investigation of differential diagnoses and the features to deduce a clinical diagnosis are reviewed. Commonly, further evaluation is required to solidify a diagnosis and may consist of genetic panel testing, imaging studies, or referrals to various specialists including ophthalmology, cardiology, or neurology. If a diagnosis can be made, the practitioner provides education regarding treatment of symptoms, prevention of complications, and ongoing health surveillance. Show less -
