My final placement in Anglicare Sydney provides me the opportunity of working as a student social worker in the Intensive Family Preservation and Restoration team. My major duties are to work closely and directly with children and families, especially parents who need child protection supports as well as children in the Out-of-Home-Care system. My day-to-day work includes conducting home visits and supervising family time visits, documenting case notes, and participating in inter-agency meetings. With a client-centric approach, I was able to develop and utilise my skills to implement positive and holistic services for clients in needs.

Working within an Agency of NSW Health, Integrated and Community Health and Youth Health. Youth Health offers health support and services for young people ages 12-24 years across the Western Sydney Local Health District Area, who are experiencing marginalised and at risk. Thriving Families NSW is a program that provides a holistic assertive case management service to young and vulnerable parents with children aged 0-5 years. It aims to provide a multidisciplinary approach to early intervention and education for young parents. Vulnerable Families offers support and assistance to vulnerable families and children. • Predominantly Research - Create a report that evaluates the effectiveness of PFM’s (Pregnancy Family Meetings), data analysis, patient satisfaction / feedback and outcome measures.• Exposure and observation of PFM’s, - Process, referrals, intakes procedures, the Three Houses model and closures of cases.• Exposure and observation of referrals for Vulnerable Families from FACS in creating Health Plans. • Exposure and observation of Youth Health Assessments (HEEADSSS) – Biopsychosocial Assessments. • Working with Thriving Families NSW and appropriate assertive case management tasks for complex needs young parents.
