Youth Advocate Case Manager
Current*Proctor individualized CASAS basic skills assessment and identify employment goals basedon career interest, ability, and personal development considerations*Develop Individual Service Strategies utilizing the PaCareerZone and My Next Moveplatforms to create a plan based on youths’ strengths and goals*Build one-on-one working relationships with participants to assist in mitigating challengesto obtaining and retaining employment*Provide career readiness and life skills classes for youth*Coordinate onsite and offsite career awareness field trips with employers and educationalfacilities*Provide comprehensive case management and follow-up to ensure positive outcomes andgoals are met as required by contract*Provide referrals to appropriate community service or social service agencies*Establish detailed documentation of all client records to maintain organized database andgenerate all project reports as needed
