Oversee the clinical team’s day-to-day functioning to ensure compliance with all program requirements. Specifically, particpates in the hiring, training, coaching, mentoring and supervising of team members, facilitate daily team meetings, to ensure all member needs are coordinated and addressed. Work to ensure clinical quality and productivity standards are met by the team by providing clinical oversight of services provided, assessments conducted and service plans implemented for all members assigned to your team.

Worked collaboratively with the clinical team to engage, educate, communicate, and coordinate care with clients, their family, behavioral health, medical and dental providers, community resources and others in ensuring that all services prescribed in the individualized service plan are implemented. Performed all case management functions associated with caseloads including outreach and follow-up, staffings, client transportation, completing assessment and service planning processes and identifying the need for further or specialty evaluations.
