Conduct comprehensive assessments of patients’ health conditions, including physical, emotional, and social needs.Develop and implement individualized care plans based on patient assessments and physician orders.Administer medications, manage intravenous (IV) therapies, and perform wound care as required.Monitor patient vital signs, responses to treatments, and overall health status, making adjustments to care plans as needed.Perform routine and specialized procedures, such as catheter insertions, injections, and diagnostic tests.Provide assistance with activities of daily living (ADLs) such as bathing, dressing, and mobility if needed.Educate patients and their families on disease management, medication administration, and self-care techniques.Collaborate with physicians, therapists, and other healthcare providers to coordinate comprehensive care.Communicate effectively with patients, families, and caregivers to ensure understanding and adherence to care plans.Arrange for additional services and resources as needed, such as physical therapy or home medical equipment.Maintain accurate and detailed patient records, including progress notes, treatment plans, and care logs.Prepare and submit reports on patient status, changes in condition, and outcomes as required by agency policies.Provide training and support to patients and families on managing health conditions and using medical equipment.Offer guidance on lifestyle changes and preventive measures to improve health outcomes.Adhere to infection control protocols, safety standards, and home care regulations.Ensure that care is delivered in a manner that respects patient privacy and confidentiality.
