-Working with the Residency Director to develop an individualized program which was unique to my role -Performing ten to twelve monthly mandatory and elective rotations of a clinical or an administrative nature-Actively participating in departmental activities Responding to all drug related needs of patients, nursing staff, medical staff and other health care professionals-Developing and implementing collaborative drug therapy protocols as a part of an interdisciplinary team Completing and submitting special projects, assignments and a manuscript-Performing scheduled work as a staff clinical pharmacist-Developing the skills, knowledge and attitude necessary for the provision of high quality pharmaceutical care

-Adapting the interview technique according to the current needs of the patient receiving their chemotherapy at Daycare-Combining pharmaco-therapeutic knowledge with a solid foundation of excellent communication and patient-interviewing skills-Explaining to the patient why this interview is conducted and how long it takes-Checking the currently administered medication, its rate and specific per-patient precautions if any -Confirming on the patient the duration of his chemotherapy cycle and total number of days -Collecting all data by documenting & filling in "IV Checkpoints at Daycare" form on HIS (Cerner) in the patient’s medical record; using open-ended questions -Illustrating the common side-effects that patient may experience and how to minimize, all necessary instructions and checkpoints during CTH administration, and all required precautions at home and immediate corrective actions or ER referral-Providing patient education and assessing patient’s medication use and compliance-Providing up to date drug information administration guidelines and recommendations to physicians, patients and nurses-Assuring that patient is taking his home medications correctly to optimize adherence-Assuring on daily basis that nurses are following all necessary instructions and checkpoints during chemotherapy administration-Assuring a good communication plan with OPD Pharmacists to provide all necessary instructions upon administering a new chemotherapy or following a new protocol-Educating nurses about all chemotherapy side-effects the patient may experience during or after administration

-Calculating compatible fluids for admixture, pre and post chemotherapy hydration, and adjunct medications -Giving patient medication history inquiries upon referral from physicians and other medical personnel -Applying patient’s proper treatment plan including chemotherapy, radiotherapy planning, and lifestyle -Revising, monitoring, and recommending changes based on information obtained from patient’s record on HIS (Cerner)-Checking chemotherapy on HIS (Cerner PowerChart Oncology) through a checking checklist specific to treatment plans, medication administration guidelines, medication dosing adjustments-Assuring ongoing performance improvement of the pharmaceutical care services provided-Documenting clinical interventions for happening events-Medication reconciliation and take home oral medications delivery for Daycare patients-Checking the appropriateness of prescribed IV supportive medications acc. to Infectious Diseases guidelines

-Synthesizing a treatment plan for any given diagnosis, which may include drug therapy and lifestyle modifications, and transition within the continuum of care settings-Identifying, resolving, and preventing potential and actual medication-related problems including untreated indications, improper drug selection, sub- therapeutic dosage, overdose, failure to receive medication, adverse drug reactions, drug interactions, and medication without indication-For any given protocol, initiating, adjusting, monitoring, and recommending changes in drug therapy based on information obtained through patient interviews, objective monitoring parameters, review of drug profiles and the patient medical record -Coordinating and interpreting laboratory monitoring of drug therapy, including serum drug levels, electrolytes, and other approved drug-related laboratory testing-Providing patient monitoring for the purpose of evaluating and optimizing patient outcomes, drug utilization and patient compliance, detecting and/or minimizing ADRs, eliminating unnecessary drug use and duplication-Checking protocols in specific to treatment plans, administration guidelines, medication dosing adjustments, required labs as they pertain to appropriate dosing, and toxicity monitoring and adjusting subsequent dosing-Providing pharmacokinetic services for Daycare patients

-Dispensing take home oral medications for both outpatient and Daycare units-Properly filling and dispensing an average number of +80 prescriptions per day-Managing drug, stock and supply inventories and performing a weekly physical medication count -Performing technical processes required to dispense medications to patients-Inspecting drug storage sites, monitoring drug supply expiration dates and ensuring proper supply levels Completing new and updating patient profiles, including lists of patient medications-Prepacking bulk medicines, filling bottles with prescribed medications and affixed correct labels
