All group work will be in line with the relevant national guidelines such as NICE guidance and that from Arthritis Research uk. • To support, advise and motivate patients to achieve identified treatment goals using a coaching and motivational approach which encourages longer term self- management.• To support patients with long term health conditions to gain independence and increased confidence to exercise within their capabilities. • To assist with telephone advice and treatment planning in line with patients’ needs as identified by a physiotherapist• Contribute to and deliver when requested, training sessions for Physiotherapists as part of the regular In Service Training programme (IST). This post will focus on strategies to develop physiotherapy colleagues level of expertise in exercise prescription/rehabilitation strategies.• Ensure all patient related information is secure and confidentiality of information is maintained at all times• Provide excellent patient care for a range of conditions, managing stressed and anxious patients with tact and empathy• Engaging with carers and relatives to support and inform them with the purpose of reinforcing the need for compliance with issued home exercise programmes.• Ensure the professional image of the Trust is maintained at all times.
