This has followed on from my earlier experience of triage, where I was responsible for allocating front line assessments following receipt and triage, to ensure the urgency requirement for the individual and referrer is met. This is work that I would like to continue pursuing and an area where I would like to continue enhancing my skills.

To provide holistic and recovery focused care for the service users on my ward, by ensuring that their needs are met. Firstly to use part 2 of the Mental Health Measure, and to help the service users identify what their definition of recovery is, as set out in their Care and Treatment Plan. With the involvement of the service user, this document will help staff and service users identify their areas of need, and provide a written document for which the service user may take ownership and joint responsibility for.It is also necessary to recognise triggers and stressors leading to the deterioration of mental state of the service users in my area. As a challenging behaviour ward, we are often called upon to produce a low stimulus environment conducive to recovery. We may also be required to intervene in times of distress which may precipitate challenging behaviour including violence and aggression, deliberate self harm and suicidal / parasuicidal behaviour. This often requires de-escalation skills, the use of the therapeutic bond and last of all, pharmaceutical intervention.Psychotherapies are also a part of the service delivered on Ward 5. Emotional coping based therapies, particularly involving service users with borderline or emotionally unstable personality traits are commonly used and a part of the qualified nursing role on ward 5. It is important that this is delivered using a team approach and therefore group supervision is scheduled for on a weekly basis, strengthening the team based approach on ward 5.Finally, responsibilities include ensuring that all forms of assessment and monitoring are maintained, particularly within my allocated service user group. This ensures regular one to one time for service users and their named nurse, to continue to identify possible obstacles exist to recovery, but also to identify strengths and protective factors that will facilitate it.
