Current● Interviewing 175 clients to obtain critical health information to ensure client’s safety.● Developing care plans to improve memory, attention, problem solving, and mobility by more than 50% for clients with serious cognitive deficits due to stroke, head injury, or neurodegenerative diseases. ● Documenting daily session notes, progress notes, evaluations, discharge summaries, and recertifications. Communicated patient progress during therapy with other members of care team, such as: nurses, CNAs, physicians, physician assistants, other therapies, administration, and families. ● Providing family training to maintain patients’ current level of function. Scheduling and conducting education with families and patients. Planning and implementing device trials and training for AAC devices.
