• Provide harm reduction education and supplies• Conduct VAT interviews and assessments • Assist with PWD applications • Connect clients to services in their community • Liaise with other healthcare professionals in the community • Arrange, plan, and escort clients to their appointments and other activities • Monitor and record clients mental and physical health

FKA: The John Howard Society • Assist clients to achieve greater independence through mentorship, skill building, and SMART goal setting• Create needs assessments, intervention plans, and service plans for clients• Ensure issues of daily living such as, employment, housing, nutrition, mental and physical health, are being monitored and are included in service plans• Collaborate frequently with external supports such as client’s family members, care providers, and other organizations • Follow a person-centered, harm reduction model• Connect clients with resources in their community• Respond to emergency situations with limited background knowledge situation and/or client • Advocate for client needs• Submit quarterly client progress reports and submit detailed daily case notes • Work independently but involve a supervisor when a situation is beyond individual capacity • Build rapport and trusting relationships with clients
