The Remote Care Monitoring program leverages technologies to keep senior adults aging safely at home. Transitional care coordinator oversees the transition of complex, and often marginalized, RCM patients from partner hospitals to home across southern Ontario by managing the client care to ensure community services and supports are in place to minimize ED visits and hospital readmissions. As a clinician passionate about holistic health and wellbeing in the community, I spearheaded the development and implementation of the RCM Active Together program. Through an innovative, volunteer-led virtual delivery model, RCM Active Together optimizes program resources while providing increased access to evidence-based physical and social wellness.
