Manage routine and acute health care needs for both long term nursing home residents and short stay rehab patients with a focus on reducing hospitalizations. From September 2014 until March 2018 I independently managed a caseload of 75-100 long term care residents and rehab patietns. I collaborated regularly with primary care physicians assigned to my patients.From March 2018 until August 2018 I continued to manage a caseload of long term care residents while also taking on the role of a clinical advisor. In this position I help onboard new staff and act as a resource for my fellow teammates. It is also expected I cover for my manager when she is away.
