1.Services the Chair of the Incident Review Committee (IRC) for Developmental Disabilities (DD) Services. Responsibilities include but are not limited to:a. Schedule all IRC meetings and serve as point of communication for all membersb. Drafting and finalizing minutes for all IRC meetingsc. Distribute investigations to all committee members for discussion.d. Coordinate any follow up necessary as a result of recommendations made by the committee2.Review all corrective actions submitted in response to recommendations made at the conclusion of a DD investigation. 3. Distribute all investigative files, and corresponding DD IRC minutes, to appropriate program directors. 4. Responsible for closure of all DD incidents (or oversight in regard to 625s and internals). Also responsible for any Corrective Action Plan (CAP) revisions (when necessary).5. Responsible for Administrative Action Reporting Mechanism (AARM) submissions to the Justice Center.6. Draft and submit plans of corrective action (POCAs) in response to statements of deficiencies (SOD) received by state oversight agencies. Also must keep an up-to-date recertification schedule for DD services7. Responsible for distribution of letters of determination (LODs) and abuse/neglect investigations once LOD is received.8. Conduct investigations, as needed, including assisting with training of new QA Specialists. 9.Responsible for supervision of the QA Coordinator10. Responsible for supervision of the QA Auditor.11. Responsible for oversight of the following areas:a. Processing of all internal reports by the QA and Corporate Compliance Administrative Assistant.

• Chair and manage Incident Review Committee process, including taking minutes, updating minutes within required time frames and sending all minutes to programs when necessary.•Manage Plan of Corrective Action(POCA) process after state surveys for DD Services division.• Develop and facilitate Incident Reporting Training for the agency programs under the auspises of OPWDD and Justice Center.• Manage state databases including IRMA, NIMRS, and AARM.• Review investigations and make recommendations as needed.• Oversee Corrective Action Plan by providing supervision to CAPS Associate.• Create and submit CAPS Packets to IRMA/NIMRS.• Ensure all abuse/neglect cases are submitted to the WSIR within required time frame• Manages investigations folder closings process.• Act as the Justice Center Liaison.• Provide training to investigators regarding investigations, and incident management.• Provide training to programs regarding CAPS documentation and verification.Initiates incidents in IRMA/NIMRS to clear que as needed• Conducts Utilization Review for a specific residential program.• Conduct investigations as needed.• Attends and participates in supervisory meetings and all staff meetings with Director of Assurance, and other Administrative staff.

• Conduct investigations involving consumers and/or employees in the DD and MH programs and complete written investigation reports as per Justice Center, OPWDD and OMH guidelines.• Submit final investigations to the appropriate regulatory agency.• Review incident reports and process incidents in IRMA and NIMRS.• Conduct admission and annual utilization reviews for MH residential and clinic programs.• Co-chair for Mental Health Incident Review Committee.• Conduct case reviews for deaths and suicide attempts.

• Developed and facilitated an agency wide training for facilitators of the Wellness groups at each site. • Facilitate on-site groups and off site activities.• Document consumer participation in groups, community meeting, and activities in AWARDS.• Assist consumers with obtaining permanent housing and develop housing resources and relationships with community organizations and housing providers.• Assist DSS with determining eligibility of referrals, conducting intake interviews and managing waitlist.• Schedule and participate in yearly utilization reviews with each consumer of the residence.• Audit charts of case managers.

• Provided case management services for caseload of 14 people diagnosed with a mental illness and/or substance abuse and have histories of homelessness.• Assisted caseload in developing the necessary skills, such as cooking, budgeting, managing appointments and medications, to move to more permanent housing and live independently.• Helped consumers apply for and/or maintain entitlements such as Social Security income and food stamps. • Performed crisis intervention, assessment, advocacy, negotiation, referral and case planning activities with residents as needed.• Maintained charts for caseload and documented progress notes, services plans, functional assessments, and psychosocials in AWARDS.• Supervised consumers in medication management and coordinated with onsite nurse and psychiatrist.
