During the post, I worked on two projects:+ a scoping exercise of nursing homes in Wandsworth - we visited 6 nursing homes to perform structured interviews, assessing organisational factors, their needs for community services, their level of access to primary care support and general practitioner advice and assessments for their residents,+ a roll-out of an NHS vanguard project from a neighbouring borough into Wandsworth - we created the specifications for the job roles for the nursing home community service team, and interviewed and placed a project coordinator for the delivery of the nursing home support team pilot project.
