Manage team of 8 Registered nurses and Certified Wellness Coaches, 1 Supervisor and 1 Research Specialist. Oversee clinical and operational outcomes that focus on performance, outcomes, satisfaction and identifying opportunities for people to make lifestyle changes that can lead to improved overall health.

Implement telephonic chronic conditions management to identified population that included disease specific and co-morbid conditions across the continuum of care in Commercial FEP and Medicare markets.

Clinical role of Registered Nurse. Responsible for reviewing clinical intake records for demographic, insurance, ICD codes, medical history, physical orders, discharge planning orders treatments and follow up instructions. Creating medical records for incoming patients and multi- disciplinary staff. Act as intake/weekend nurse for patient/family questions and Physician outreach.
