The Outpatient Coordinator is responsible for providing service coordination to adults with chronic mental illness, substance abuse, and physical health conditions. The goal of carecoordination is to reduce unnecessary contacts with emergency departments and unnecessary inpatient hospitalizations while removing barriers to outpatient services and healthy outcomes.

Intake Coordinator has the general responsibility of providing professional evaluation and triage of patients to the most appropriate level of psychiatric service. Intake Coordinator evaluates and coordinates psychiatric intakes, during their shift, as needed. Face-to-face interviews are conducted in the Emergency Department. The responsibilities will entail crisis management, intake and referral for placement of patients within the Psychiatric Programs, case coordination with community referral sources, and utilization review for initial placement of patients.

Assists in coordinating essential program activities such as activity groups and specialty groups. Works closely with all other clinical staff (Activity Coordinator, Social Workers, Nurses) to conduct various process and specialty groups within the patient program. Is integral in assuring that the patient program schedule is carried out as planned by the staff team. Performs various duties assisting staff nurses in the psychiatric and physical treatment and care of patients in accordance with established methods and procedures of the hospital. Interacts closely with patients remaining engaged in the various activities and events that patient's experience throughout the treatment day. Interacts with patients in a therapeutic manner at all times and assists other staff to also maintain appropriate boundaries. Assists patients to perform routine activities of daily living.

Conduct market research in conjunction with preparing and packaging property profile presentations for target market.
