I assist with running a weekly day program for participants between the age of 16 and 35, the day program runs every Tuesday and Thursday and works towards participants' independent living skills as well as their social and community participation.

My current field education placement is at Ability options within their Work Ready program. The program aims to equip people with a disability with the skills, knowledge, and real-world experience to enter or re-enter the workforce with confidence. The work ready program runs weekly workshops 2 days a week on a variety of life skills topics which participants can attend if they wish. The program also allocates each participant with a Work Ready consultant and they are expected to do a weekly 1:1 session, this time is used to work on any skills or areas the participant would like to work on. This allocated time can also be used for work experience in which the work ready consultant can provide 1:1 job support to the participant, the program works with the community to be able to provide the participants with as many work experience opportunities as possible to be able to get them out in the community and at a level where they are comfortable to look for permanent employment.

-I assist a variety of different members within the community with various disabilities and health concerns with daily tasks and assist members of the community to gain better access to resources and support.-I’m an advocate for my clients and assist with forms and applications, transportation to appointments and everyday tasks including housework.

-Attendance at daily ward round meetings -Attending meetings relating to patient care such as 'End of Life' meetings, where all deaths in the ED/ ICU are reviewed -Shadowing social workers across various clinical areas -Attendance at professional development training sessions -Direct patient work including conducting psycho-social assessments, recording documentation in the medical file, supporting families -Liaising with stakeholders including Healthcare Interpreter Service, SIRA (CTP matters). contacting DCJ with the supervisor -Critical reflection/ reading of written material to expand on theoretical knowledge/ skills -Simulation sessions facilitated by field educator
