Occupational Therapist
Current• Provide skilled occupational therapy assessments to patients with a variety of medical needs where patients are homebound. • Develop comprehensive treatment plans and provide intervention to address occupational therapy needs including ADL/IADL needs, fall prevention techniques and recommendations on home modifications to improve in safety within the home/ILF/ALF.• Engage in timely, detailed, and patient specific documentation on care plan, progress, goals and discharge planning. • Educate patients and families on fall prevention strategies, energy conservation techniques, compensatory treatment strategies to improve in ADL/IADLs and safety within the home. • Case Manage patient care needs while following Medicare A chapter 7 guidelines including: SOC/ROC OASIS, Recertification, progress visits, End of Episode/Discharge planning, obtaining and communicating with staff and physicians for multi-disciplinary referrals, knowledge of LUPA guidelines, hospice/palliative care planning, medication reconciliation and timely NOMNC
