CyncHealth is a statewide Health Information Exchange designed to share clinical and administrative data among providers in Nebraska and neighboring states. CyncHealth’s purpose is to achieve health care transformation through community betterment collaboration while protecting the security and privacy of medical information.Highlights include:• Wrote grants that supported organizational mission of addressing health disparities and was awarded $80,000 in total grant funding• Assisted with the development of an interoperable social determinants of health (SDOH) technology platform for health systems in Nebraska/Iowa• Utilized clinical data sets to analyze health disparities for the health information exchange (HIE) using Excel, Tableau and SAS• Worked with community-based organizations and health providers to identify barriers in health equity and implement strategies to mitigate barriers

The Department of Health and Human Services team contributes to the lives and health of Nebraskans every day.
