Adult Mental Health Case Manager
CurrentConducts risk assessments and mental status exams • Supports the intake process and assesses client needs by performing initial screening of referrals Collaborate with psychiatrists and counselors to ensure clients mental well being and correct treatment plans are in place • Provides follow-up care and services as required• Makes necessary arrangements for clients to be admitted to hospital when required, including on an involuntary basis• Facilitates client utilization of appropriate community services and rehabilitative programs through activities• Maintains client records by recording information such as client assessments, charting client information via PARIS• Participates actively and collaboratively in team planning and team meetings• Carries out responsibilities in accordance with health and safety requirements• Provides treatment and support for complex situations• Ensuring clients receive injections at the correct time to support their mental well being • Advocate for clients by participating in meetings with Psychiatrists and clients• Practise harm reduction techniques
