I have worked in Case Management in South Africa for the last 17 years. I worked at a medical aid administration company doing Intensive case management for 16 years and at a private hospital as the hospital case manager liaising with medical aid funds for 2 years. My previous employment included authorising of admissions to hospital and following up on those, as well as emergency admissions, to ensure that the appropriate care was being rendered and that the patients were not being over serviced. That appropriate ward movements were charged and associated providers treating the patient's were clinically valid. I ensured that their treatment, if appropriate, was authorized. Ongoing liaising with the next of kin or family members. Any prosthetics were approved if appropriate and the most cost effective All alternate care was also authorized if clinically appropriate i.e. Stepdown care or rehabilitation care approved, out-patient physiotherapists visits, appropriate follow up blood tests, speech therapy and any other appropriate care or treatment as requested. A follow up visit or telephone call was scheduled for all major admissions. All appropriate documentation was completed and kept up to date. Care-plans and risk assessments done.Ongoing evaluation of the member's clinical condition was done and recorded. Referrals to other associates made as was clinically appropriate i.e. If newly diagnosed with cancer then a referral sent to the Oncology disease manager for enrollment on the Oncology program and approval of treatment and ongoing support and assistance. A move to "Wellness" was being rolled out and I had received in-house training regarding same.
