This position ensures that incoming applications and documents submitted by potential clients forprogram eligibility are complete and legible. Verifies that all required documents are received andproperly completed. Ensures that all documents are properly linked to the correct person or familyassistance group. Occasionally, contacts the client by email, telephone, or fax to clarify submittedinformation or obtain additional information for program eligibility assessments .Experienceinteracting with clients in a social services environment gathering and providing information anddeveloping case plans. Duties and Responsibilities to include. Determines proper handling of incoming requireddocuments, researching to find the correct document type, client and/assistance group to ensurethat all documents are linked to the correct client or family assistance group and to verify that alldocuments are acceptable per agency policy. Reviews applications and income, asset andresidency documents submitted by applicants to determine the applicant's eligibility for theHillsborough Health Care plan. Interviews clients via telephone, email, or other writtencorrespondence. Review documents submitted by the clients for program eligibility assessment.Follow up with clients to ensure application is properly completed and all required documents aresubmitted for eligibility determination.

Gathering medical records and patient information , taking ocular measurements, both anatomical and functional, administering or preparing medications, providing eye care and contact lens information to the patient, maintaining and cleaning ophthalmic instruments, assisting the ophthalmologist and Optometrist in basic procedures, visual acuity measurement, refractometry, tonometry, measure pupil size and reactivity, test and measure eye muscle function, Visual field testing, NCT testing, Topography , OCT, Fundus Photos, MedPros Update, Using SRTS to order glasses, Assist in frame measurement and/or selection of spectacles to assist patients, Create reports using the Spectacle Request Transmission System (SRTS) to track optical orders, Perform medical data entry with automated medical record system such as, Armed Forces Health LongitudinalTechnology Application (AHLTA) to document patient care. Order Inventory and supplies, andmaintain stock. Uses multiple office software with varied functions to produce a wide range ofdocuments, formats. Provide facilities with supplies and material including, but not limited tosterile and non-sterile medical supplies, instrument sets and equipment. Maintain stock forprimary and secondary inventory locations for all clinical and administrative services, Pull stockand material, replenishes stock inventory.
