As a care manager my responsibilities consist of the following:- Reviewing referral information and existing assessments prior to coordinating the initial team meeting.-Assist in providing services for youth with mental health and developmental disabilities.-Coordinate and facilitate a process of working with children and families utilizing strength-based team approach to ensure youth and family needs are addressed and met.-Monitor the child and family team (CFT) and facilitate its operation, ensuring coordination with the Family Support Organization.- Produce outcomes that consistently meet standards, expectations and best practices.-Connect youth to available resources in the community including but not limited to housing, rental assistance, social service entitlements, domestic violence, outpatient mental health and substance abuse treatment facilities, food pantries, etc.-Complete required training and certification including strength and needs assessment, wrap around, cultural competency, etc. -Document all contact, interventions, and status updates in the clinical records following established procedures.-Develop transition plans for the child and their families to move from the CMO to other services.-Monitoring of strength and needs assessments, crisis plans, family vision and ISP’s with the Child Family Team.-Complete Presumptive Eligibility application for newly enrolled youth and monitor on-going eligibility-Responsible for quality of documentation and monitoring through peer reviews.-Attend family court for hearings with youth involved with the Department of Child Protection and Permanency(as requested).- Maintain confidentiality of youth and families and follow HIPAA regulations.-Transport youth and families as needed- Responsible for administrative support to teams as needed and on-call coverage on rotation basis
