During this Advanced Pharmacy Practice Experience I was able to shadow and work with pharmacists spanning a multitude of specialties, observe hospital pharmacy operations, deliver medications to different hospital departments, perform IV preparations and batching, and be actively involved in the emergency department under pharmacist supervision. My responsibilities included anticoagulation monitoring, pharmacokinetic consults for vancomycin dosing, IV to PO monitoring, and high risk medication reconciliations.
