 Working directly under supervision of Wellness Director of Nursing, to aide and assist with all components of resident care. Working as a certified nursing assistance or medication aide as needed.  Daily communications with families to ensure lines of communication are open, resident daily needs are met, and any family concerns are address.  Orient and train new staff in building.  General office duties, i.e. filing, report preparation, answering phones etc.  Setting outside consultations for residents, working with primary physician for referrals for resident needs, scheduling transportation for visit (both return and departing, and ensuring resident leaves with all appropriate paperwork necessary for visit) On average over 50 appointments (initial consultations, follow-ups and surgical) per month, for 90 bed assisted living facility.  Development of comprehensive employee assignment information sheet. o Revised employee sheets to include all pertinent information regarding resident, making floor changes due to staffing issue easier to transition. Assignment sheets reflect important factors such as if resident is a diabetic, is a DNR, wears and wander guard, uses incontinent supplies, has a special diet, and what adaptive equipment is used.  Directly responsible for scheduling in house consolations visits. o Revised number system to ensure residents are seen in based on urgency order, decreasing wait time for residents to be seen by Doctor thereby reducing resident complaints. o Making family participation in consultations a priority, informing families of upcoming visits, and setting appointments based on their availability during the time frame of the consultation visit. o Making sure residents are scheduled in correct time frame allowable by Medicare/Medicaid for specific services. Maintaining compliance with all healthcare facility, HMO and insurance requirements.

Assist hospice care residents will all aspects of daily care, bathing, feeding, incontinence, and dressing in accordance with established care plan and family requests. Travel to various locations, nursing homes, private residence and assisted living on an as needed basis. Report care, charting and vital signs back to nursing staff on a timely basis.Primary Care Provider

Shared joint responsibility with family in care of 80 year old, bedridden grandfather suffering from dementia. Assist family with all aspects of daily care, hygiene, meal preparations and dressing. Accompany patient while being transported to doctor appointment.Assist resident with ambulation, transfer from bed to wheelchair, as established by care plan, and physical therapy plan. *job terminated due to patient transfer to Nursing home.
