Oversees the development and implementation of prioritized ambulatory and acute care clinical pathways and quality programs. Critically evaluates and refines the process of pathway development, implementation, and monitoring by collecting and acting on feedback from stakeholders and organizational leaders. Partners with Medical Director of Population Health to develop and champion population health strategy across the health system.

•Oversees the development and implementation of selected primary care-based clinical pathways as agreed upon and prioritized by Q&V leadership and the Q&V committee •Collaborates with the BPT and PCMH teams to develop, implement, monitor, report, and improve pathways •Critically evaluates and refines the process of pathway development, implementation, and monitoring by collecting and acting on feedback from stakeholders •Fosters and maintains collaborative relationships throughout Primary Care clinical and operational teams as well as other Swedish functions (e.g., Information Services, Decision Support) related to quality initiatives and performance feedback •Develops an understanding of pathway initiatives across Swedish and Providence and leverages/adapts preexisting work within Swedish when appropriate •Integrates an understanding of externally mandated quality metrics into the development and implementation of pathways •Reviews and evaluates the application of data to measure structure/process/outcome continuum to improve quality •Collaborates in development of policies and procedures related to quality improvement as needed •Conducts employee performance evaluations for supervised staff. Regularly reviews and supports employee goals and development plans •Assists in hiring new employees and coordinating subsequent training and orientation programs. •Provides supervision and leadership of Q&V Quality Improvement Specialist

- Provided care coordination to Sea Mar patients with chronic illnesses such as diabetes, hypertension, and asthma as well as mental health concerns among other health issues. - Led and facilitated the monthly meeting of a “Care Team” including medical providers, clinic management, and support staff performing Quality Improvement activities with the goal of improving clinic outcomes and patient care. - Worked with team members and served as a clinic champion toward accreditation as a “Patient Centered Medical Home.”- Served as an on-site trainer and mentor for an Americorps volunteer member working in the clinic and participated in the hiring process for a new Americorps member.- Taught brief education for patients after provider visits regarding chronic illness, nutrition, self management, and goal setting. - Teamed with patients to determine and set reachable goals for their own health and make referrals to health education and community resources.
