Village Concept Program is a state awarded grant project -In charge of researching, planning and gathering inputs from senior citizens in DeKalb County -Work with local strategic partners, elected officials office, regional Area Agency on Aging (AAA), and GA State Aging Division

(1) Developing Logic Models which aim to evaluate the quality and outcomes of senior services provided by DeKalb County, and services delivered by contractors(2) Work with Operation Team and in charge of developing operation plan that establishes guidelines to best provide all senior services in a unified, synergistic manner(3) Submitted a proposal for SFY 2013 Georgia Department of Human Services Division of Aging General Mini Grant for Innovative Programming -Address the food security issue and food desert among DeKalb County senior citizens, and proposed a systematic and integrated solution(4) Health and Wellness Program Specialist -Coordinate two evidence based programs in DeKalb County: Chronic Disease Self-Management Program (CDSMP) and Sickness Prevention Achieved Through Regional Collaboration (SPARC) -In charge of program planning, implementation, marketing and community outreach
