Assist patient and families with adjustments to illness and/or injury by providing emotional support, coordination of care and connecting patients/families to appropriate resources in the community.-Work closely with multidisciplinary teams to support medical recommendations while advocating on behalf of the patient/family for the best outcome of the patient. -Assist patients with complex family dynamics, financial and housing concerns, lack of insurance, limited or no support system, homelessness and inadequate coping skills.-Work with patients, family members and friends to assist with socialization, community integration and decision making regarding discharge planning.-Participate in multidisciplinary rounds and inform and update medical providers of the patient's psychosocial, mental health concerns, or chemical dependency issues.

• Provide therapy to clients ages 6-55 years old, with presenting issues including but not limited to anxiety disorders, depression, mood disorders, personality disorders, sexual abuse, suicidality, life transitions, grief and loss, PTSD, parent/child and marital conflicts, pornography addiction, blended families, missionary life transitions. • Complete all client paperwork for each case including: individual treatment plans, functional assessments, diagnostic reviews, termination summaries, and established objectives with clients. Consulted with clients' parents and ecclesiastical leader, when permitted and appropriate to coordinate care. • Diagnostic Tools used with clients included: OQ-45.2 (Outcome Questionnaire), MOQ (Marital Questionnaire), Y-OQ 2.01 (Youth Outcome Questionnaire), Beck Depression Inventory, Burns Anxiety Inventory. • Experience with assessing, evaluating and diagnosing client's behavioral, cognitive, emotional and interpersonal symptoms using the DSM-V (Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition).

Hospice Social Worker• Worked with persons who have a life expectancy of 6 months or less, who were receiving Hospice care. Client population also included patient's family members. • Worked with clients and families whose diagnoses vary. All clients had a medical diagnosis that was life limiting to 6 months or less. Some clients also had mental health diagnoses including anxiety and depression. The presenting issues for many clients included anxiety and fears around coping with terminal illness, loss and death. • Involved in assisting the client in treatment of their mental health once entered into the Hospice program as well as assist in crisis intervention. This included ongoing assessment and treatment plan reviews. Consulted with team members on staff and clinical supervisors about assessment, diagnosis and treatment planning. Assist clients and families with community resources as needed.
