At Our Mustard Seed Nutrition, we strive to help our clients live a healthy and nutritious lifestyle. We not only focus on the body, but incorporates feeding the soul as well. Our name specifically is based on the bible parable of the mustard seed. Just as the parable states, the mustard seed seems small and insignificant; but when it receives love and nourishment, it grows into a large wonderful tree. - Manage and coordinate 24+ chapters (teams located on different school campuses)- Educate membership on accreditation items and how to plan to meet their strategic goal.- Assist in the development of chapter specific goals that are attainable for the progression of chapter growth.- Facilitate meetings and educational calls on the accreditation standards and parameters for a chapter to fulfill the different levels requirements.- Review and assess documentation and programming completed by chapter.

Providing medical nutrition therapy and nutrition education to pediatric patients and families. Work in multi-disciplinary teams to support in the healing process of the patients. Examples of types of patients seen: hematology/oncology, behavioral health, post-surgical, and feeding tube dependent.

Provide medical nutrition therapy and nutrition education to child, women, and families affected by HIV/AIDS epidemic. Providing support groups and educating on how to overcome barriers to be succusseful in their health and personal goals.• Assess all pediatric, adult, and geriatric patients within the Comprehensive Family AIDS Program (CFAP) for nutritional risks and execute medical nutrition therapy where risk is identified.• Develop and execute nutritional components within the coordinated patient care plans and collaborate with the interdisciplinary team.• Develop nutrition education materials for children and adults with education level variations to ensure patient comprehension and application.• Develop nutrition documentation templates and patient self-assessments.• Educate interdisciplinary team on current pertinent nutrition-focused information and research.• Develop policies and procedures for nutrition services in CFAP.• Participate in the development and execution of the transition of care policy and procedure for adolescents transitioning into adult care.• Facilitates quality improvement projects for enhanced patient centered care.• Develops and executes nutrition support groups, health fairs, and coordinated programming for the patients within CFAP.• Facilitate partnerships with community programs, nutritional supplement companies, educational material companies, food industry, and other pertinent members in the patient-centered care process.
