As a Transitional Care Manager (TCM) I was able to timely and efficiently capture requirements needed for our providers to see patients within the timeline for Hospital Follow up Visitis as well as connect with patient's or their loved ones within 24-48 hours post hospital discharge.As a caregiver of someone who was emergently hospitalized and underwent emergency surgery, I understand how scary and confusion it is to be discharged home or to a skilled nursing facility (SNF) with minimal answers to the inevitable questions that pop up. As a TCM, it was my responsibility to capture the information needed for a proper TCM contact call as well as listen to patient's needs and questions and if appropriate (through protocol) answer those questions or connect them with the appropriate level staff member to help them as well as schedule them with their PCP within the time frame needed for hospital follow up. This was a truly rewarding position that I held.As a Longitudinal Care Manager, I had the rare and special opportunity to act as a liaison between a patient and their full healthcare team as well as set goals and be an accountability partner for patients to reach their ultimate healthcare goals.
