-Implement appropriate strategies and health education programs to enhance overall wellbeing and health care outcomes of members, ensuring appropriate services are being obtained and utilized.-Develop a one-to-one relationship with members and providers.-Assess members for wellness education and disease management, educate members in condition specific program based on condition specific National Standards of Care.-Encourage members to utilize appropriate resources and web-based tools available to them to access information regarding their condition as well as track progress and develop medical record.-Conduct concurrent review, discharge planning, and alternative treatment programs for chronic or catastrophic cases to provide medically necessary and cost-effective services.-Consult with physicians, providers, members, and other resources, as appropriate, to assess, plan, facilitate implementation, coordinate, monitor and evaluate options and services required to meet an individual’s health needs, using communication and available resources to promote member wellbeing, quality health outcomes, as well as cost effective outcomes.-Discharge care coordination, complex case management, and pre-admission/post discharge follow up.-Provide education, counseling and referral to other resources to minimize re-admissions and emergency room episodes.-Personalize outreach based on attitudes/behaviors/ risk assessment. Outreach to providers to coordinate member care or notify of new risks or conditions.-Educate and monitor on medication safety and reinforce provider post-operative instructions of care, diet activity levels, etc.-Encourage members to utilize appropriate resources available to them to access information regarding their condition as well as track progress and develop medical record.-Assure effective and appropriate interface with internal divisions and external sources as appropriate.

-Conducts the initial physical assessment of patients and their family.-Coordinates the preparation of patient care plans.-Conducts periodic home visits to the patients based on the patient’s level of need.-Prepares patient care notes and ensures that medical charts are up to date through Netsmart Application.-Contacts the attending physicians with requests for orders or changes in orders.-Responds to patient and family requests in emergency, and crisis visit situations.-Demonstrates knowledge and skills necessary to provide care appropriate to the age of the patients served.-Demonstrates interventions appropriate to the age of the patients.
