Working closely with a Consultant Nurse, GP Surgeries, Community Teams, other support networks, Governance and Quality Improvement Teams. The goal is to listen to the service users’ needs and provide a care package that offers physical medical care, but also considers mental health needs, social care and complimentary therapies, plus promote self-management with goal setting. Responsibilities include coordinating all the above network, setting goals, managing budgets, effective stakeholder management, negotiating and sourcing funds to purchase up to date medical equipment and working with risk, compliance and quality improvement representatives.
