Extensive field work, providing on-going service coordination to a caseload of 45 individuals, ensuring the delivery of quality, person-centered care.Schedule and actively collaborating with the Individual and the Interdisciplinary team to conduct meetings and assessments to ensure the development of a comprehensive Life Plan that reflects the person’s needs and desired life goals.Implement, update, and monitor Life Plans and facilitate individualized Life Plan reviews and approval processes at a minimum of every six months or as needed.Connect Individuals with community services and supports, housing, social services, and family services. • Respond to MTA Customer email complaints, commendations, suggestions and requests using the Rightnow database• Input finalized reports of consumers complaints into the transit excel database• Prepare Employee commendation acknowledgments using the Department of Buses database (DOBIC)• Input train delay letters requested by MTA customers and send response letter via email or postal mail utilizing transit database system• Occasionally will call consumers for a follow-up to letters or for accuracy of reports• Basic clerical duties such as typing, filing, faxing, scanning and mailing
