As a Care Navigator I was a part of a 9 person team that implemented a new way of working in a cluster of GP Practices in Central London.I worked closely with commissioners and providers in order to bridge the gap between health and social care. I was also responsible for facilitating and leading community based MDT meetings as well as Outreach Pediatrics Clinics. My main roles and responsibilities included:Support GPs to ensure that personalized health and social care plans are planned appropriately and monitor their progress. Provide advice on the management of a patient’s pathway to clinical staff.Organized and managed monthly Multi-Disciplinary Team (MDT) case conferences to foster joined up working with various healthcare professionals. I was able to gain first hand experience of a caring environment as well as taking part in various exercises during the course including CPR training. I gained first hand experience of some of the issues and challenges facing the NHS. I acquired valuable insight in to the day-to-day role of a medical professional and gained an understanding of the amount of work and teamwork that goes into patient care.
