She also developed treatment plans, co-facilitated groups, and attended weekly multidisciplinary staff meetings. Med/Surg- Team 7: As a medical social worker, Ms. Schuller provided discharge planning services for a total of 64 veterans. Within 48 hours of hospital admission, she would perform a comprehensive intake assessment, and provide quality resources and referrals. Ms. Schuller regularly attended interdisciplinary rounds (IDR), where she provided updates to the medical team about the veterans needs from a holistic perspective. She would also assist the veteran in completing Advance Healthcare Directives. Ms. Schuller frequently provided brief crisis intervention for the veterans and their families, and helped them to determine the least restrictive environment for medical optimization. A majority of her time was spent preparing packets and facilitating nursing home placements. Show less
