• Provision of rehabilitation and education for patients following post acute injury, orthopaedic and general surgical intervention, neurological conditions including stroke and Parkinsons disease, falls and general.
• Targeted individualised patient centred collaborative goal setting including behavioural modifications to achieve goals.
• Collaboration and effective communication with a multi-disciplinary team (PT, OT, SW, SP, Allied Health Assistants, Nursing, Dietitian and Rehabilitation consultant) to achieve maximal patient centred outcomes.
• Weekly participation in case conferences with GPs, nurses and other allied health professionals for evaluation of goals, discharge planning.
• Liaise effectively with external services (physios, podiatrist, gait aid service, SP, OT, Dietitian, residential or respite care) for continuation of the holistic care after discharge from hospital.
• Consistent high quality documentation & statistics recording.
