As a QIDP I collected annual evaluations for a resident from a psychologist, social worker, Director of Nursing, Speech Language Pathologist, Occupational Therapist, Physical Therapist and a dietitian. I reviewed the evaluation’s recommendations and led a meeting with all professionals, resident and family and coordinated to make a year plan for the resident to encourage their personal growth and independence. I wrote comprehensive functional assessments, social-sexual assessments, Elopement risk assessments, Discharge Plans Behavior support plans and individual goals for each resident (20-40). I worked directly with the residents ranging from 4 years old to 50 years old and taught direct care staff how to help residents learn to be independent in their daily living skills, vocational skills, social skills, behavioral skills, budgeting skills etc. As a QIDP I was an advocate for the residents and made sure they were receiving the respect, care and resources available to them. I took pride in my abilities to maintain a healthy and appropriate relationship with each resident by spending quality time with them, listening to their concerns and providing all that I could to help them in every area of their life. My personal goal as a QIDP was to be a voice for the residents especially because some could not voice their wants and needs.
