• Assessed the needs of teens preparing to turn 18 and exit the foster care system •Built positive and healthy relationships for teens by searching through all of the foster youth's DCF files and running government background searches to locate family members or friends•Collaborated with case workers, educators, coaches, parents, permanency specialists and independent living coordinators to ensure child has a strong support team •Visited with older teens on a weekly basis to discuss future plans, resumes, healthy relationships and personal strengths and weaknesses•Facilitated group activities at post-adoption support group meetings for children under the age of 14
