Economic Mobility Coordinator, Streetcred- Pediatrics Department
Current- Build capacity of the StreetCred public health initiative to prescribe patients and families access to government services and economic opportunities by offering financial resources and tax filing as a part of a new model of holistic, primary medical care-Improve systems, workflows and policies for financial navigator recruitment, training, and management inthe pediatric clinic as part of the economic bundle of financial services.- Collaborate with the pediatric clinic operations teams to embed education about and connection toStreetCred services into the regular clinical workflow specifically for financial navigators, patientnavigators, medical assistants, providers and administrative staff.-Refine the data monitoring system to track services provided to financial bundle families with the goal ofreaching as many qualified families as possible while ensuring fidelity to the program model.- Lead, train, and direct volunteers to coordinate and implement a new patient care model that focuses on improving economic mobility and accessibility
