Patient Care Assistant ( Medical Administrative Assistant)
Current• Provides first point of contact and delivers timely and accurate service to all patients, service providers and community partners.• Obtains and enters patient information and service details into the patient database;• Obtains appropriate consent in accordance with the privacy policies.• Processes placement applications and sends out notifications.• Directly responds, or coordinates responses to requests for information from patients (e.g., community resources, placement and scheduling issues, application process, etc.)• Prioritizes and manages the Care Coordinator’s calendar, sets up appointments on the Care Coordinator’s behalf and resolves scheduling conflicts.• Retrieves statistical data and ensures accuracy, forwarding reports and taking action as needed.• Accesses, generates, and follows-up with team risk management and quality assurance reports utilizing the appropriate software tools (e.g. ORTS (Occurrence Reporting Tracking System).• Coordinates the ordering of medical equipment and supplies.• Coordinates and communicates service referral information to provider agencies; triages voicemails and paperwork.• Maintains a daily focus on patient safety.• Reports incidents or near misses to appropriate management by using ETMS, an organizational adverse event reporting system;• Encourages and supports patients and families to actively participate in their health care. • Advocates for safe, quality patient care by making safety improvement suggestions and promoting safety initiatives;• Complies with the HCCSS, South West Code of Conduct, Occupational Health and Safety Act, applicable regulations and procedures,• Keeps alert to possible job hazards and takes prompt action to eliminate unsafe conditions;• Participates in health and safety orientation, education and training and stays informed about safety requirements;• Advocates for a safe work environment by making safety improvement suggestions and promoting safety initiatives
