Independent Living Transition Planner -Interview and assessment of youth ages 14-21, who are in placement, to develop yearly transition plans, according to the John H. Chaffee Foster Care Independence Act guidelines.-Work in an assigned regional office as a transitional and independent living resource for caseworkers of Allegheny County Office of Children Youth and Families.-Facilitate teaming meetings that are unique to the needs of each individual youth.Independent Living Caseworker, Allegheny County Office of Children Youth and Families:-Carry a caseload of youth ages 16-21 who are active with Allegheny County Office of Children Youth and Families and also referred to Independent Living programming.-Visit youth in homes and placement every 30 days.-Prepare court documents and testify in Juvenile Court.-Work closely with Juvenile Probation Officers and law enforcement.-Refer youth and families to agency and community based services.-Investigate General Protective Service referrals as necessary.-Coordinate and facilitate conferencing and teaming meetings with families.Intake Caseworker, Allegheny County Office of Children Youth and Families:-Receive and investigate Childline and General Protective Service referrals.-Work with families to ensure safety or to mitigate risk.-Assist families in crisis intervention.-Schedule and ensure forensic appointments.-Work with law enforcement and medical professionals in the community.
