I have a passion for serving patients and promoting evidence-based, patient centered care. As a practicing ambulatory care pharmacist and board certified pharmacotherapy specialist, I have extensive experience providing chronic disease management for patients with complex care needs. I also enjoy collaborating with various stakeholders to affect change on a larger scale through program development and population health initiatives. In my current role, I practice under a collaborative agreement to serve patients with uncontrolled chronic disease states who are unable to achieve treatment goals through standard of care. My role is dynamic serving as a prescriber, pharmacotherapy content expert, drug information resource for providers, and advocate as well as educator for patients. I also oversee the development of ambulatory care pharmacy initiatives to increase access to care, improve quality measures and reduce gaps in care.
