Responsible for documenting on residents, mds, careplans, phq, 9, bims, assessment, social history and admission assesment, and progress notes as required by state and Federal Guidelines. I also do behavior audits.

Conducted and planned all educational meetings on and off the facility grounds, i. e., family council, senior living facilities, etc.
