I work as a PA in adult medicine at a new concept clinic Park Nicollet opened using a new approach to delivery of primary care with less reliance on in-office visits. SmartCare utilizes team-based care for in-person, phone and virtual visits with a goal of providing excellent patient-centered care in a cost-efficient manner. There is a heavy emphasis on quality, access, and patient satisfaction.

Worked independently at a walk-in clinic in a retail setting caring for adults and pediatric patients with acute health problems (pharyngitis, URIs, ear concerns, coughs, UTIs, skin rashes, travel health, etc). Performed health screenings and provided immunizations.

I saw new and established adult and pediatric patients in the outpatient setting. Performed microscopy for ear debridement. Removed nasal splints s/p septoplasty. Saw the majority of the practice's post-ops. Utilized rigid and flexible laryngoscopy. Performed sleep apnea consults and determined CPAP titrations.

I acted as the primary care provider to an independent panel of patients in the family medicine department of a large multiple-specialty group clinic. I performed health maintenance exams in keeping with constantly evolving screening guidelines. I took histories, performed physical examinations, ordered and interpreted diagnostic tests such as laboratory tests, x-rays, EKGs and ordered advanced imaging when appropriate (CT, MRI, ultrasound), prescribed medications and performed procedures. Procedures included cryotherapy of benign and precancerous lesions, skin lesion biopsy/excision, I&D, and removal of foreign bodies from skin. I performed GYN exams and contraception counseling, performed preoperative examinations, treated acute illnesses and managed chronic disease including hypertension, hyperlipidemia, diabetes, asthma, COPD, depression, anxiety, and fibromyalgia among others. I consulted with my MD colleagues regarding more complex patients, and consulted with or referred to specialists when necessary. I utilized Epic EMR and Dragon voice recognition software, and participated in ongoing quality improvement projects, Health Care Home, and Accountable Care Organization (ACO) meetings.
