The primary focus of this role was to ensure the differing needs of the community were met efficiently in relation to COPD patient management, across all the PCT’s within Birmingham and the Black Country Strategic Health Authority.This involved assessing the availability of specific COPD services and the effectiveness of each of these services. This was to enhance the standardization of care across the region in relation to the care and services for COPD patients.Working with clinical colleagues and cross-functional teams to improve the way in which services are delivered. Creating an environment where nurse leads met and shared best practice and created resources such as patient information booklets to increase the understanding and self-management of the disease.Introduced and lead on a national Met Office health project created specifically for COPD patients. This project led to the introduction of self-management plans to hundreds of patients as a pilot to prevent as many exacerbations of COPD symptoms amongst patients as possible during the colder climate. The outcome was to prevent as many hospital admissions amongst COPD patients as possible, which was ultimately achieved. Establish effective mechanisms for supporting ideas generation and exchange, ensuring the experience and requirements of clinicians and patients affected by COPD are at the centre of the decision making process. Produce regular reports, updates and presentations on the progress of services in development to present to the board.

East Birmingham

To develop and monitor a specialised respiratory GPwSI clinics across East Birmingham to enhance the standard of care of COPD patients and to increase the awareness and prevalence of the disease. Responsible for the project management ,recruitment and monitoring of a clinical team working across an interface of two health economies which delivered a multi disciplinary approach to care within COPD clinics in primary care in numerous locations. Analyse and interpret demographic and admission data to Identify potential hot spots of undiagnosed patients and supporting a new pathway for identifying and treating patients. Undertook regular audit and monitored any changes. Achieved national BTS award for the team. Experience of GP clinical systems and the composition of business cases.Identifying and delivering the training needs of the clinical team. Producing patient and clinician pathway standardised proforma / assessment documents
