10-week final placement in the community setting with National 360. During this placement, I have assisted clients funded through various bodies, including NDIS, TAC, aged care and privately funded. I have a majority of the paediatric caseload, but I also have been exposed to a range of client populations, including young adults, adults and older adults. My roles include:-Conducting standardised and non-standardised assessments, such as Initial assessment, functional assessment, upper limb assessment, apraxia screen, Montreal Cognitive Assessment (MoCA), - Running regular sessions with clients during home visits. - Working in a multidisciplinary team that includes physiotherapist, dietitian, social worker and allied health assistant- Taking progress notes after each individual session with clients- Participating in weekly handover meetings - Contacting clients and other health professionals via phone calls- Assisting clients in applying equipment through Geat2go and SWEP application

Sub-acute project management placement. Being a project manager- my roles include: - Researching the evidence-based practice of working with lower limb amputees at sub-acute hospitals. - Creating a guideline for occupational therapists to work with lower limb amputees at Monash Health. - Providing recommendations on designing a patient handout bundle for lower limb amputees at Monash Health. - Communicating with project sponsors regularly- Implementing data collection at the sponsoring agency- Preparing and presenting the project development
