Lead Pharmacist - Anticoagulation Safety
Current• Lead the training and development of pharmacists rotating through the anticoagulation clinic• Secretary for the Trust Thrombosis and Thromboprophylaxis committee• Member of the Trust medicines safety committee• Member of the senior surgical pharmacy team providing a clinical ward service, and support pharmacy staff on managing anticoagulation related queries• Responsible for reviewing and updating the Trust guidelines in accordance with evidence based research and national recommendations• Responsible for reviewing the anticoagulant related incidents reported within the Trust, and identify trends and develop strategies to prevent similar incidents occurring• Supervise undergraduate pharmacist research projects within the field of anticoagulation• Line manager and professional tutor• Project and change management lead for the way in which anticoagulants are managed, include:o National low molecular weight heparin shortage November 2018 – February 2019. Performed at a Trust strategic level ensuring contingency plans were in place, patient risk was mitigated and effective communication was disseminated to the end users.o A quality improvement project on heparin infusion safety reviewing the transition of unfractionated heparin infusions from a paper based prescribing and monitoring system to the Trust electronic prescribing and monitoring system. The project concluded the change provided improved care quality and safety. The project was presented at the UKCPA conference 2019 and was shortlisted for best postero Direct oral anticoagulant (DOAC) prescribing: Change management processes within the Trust to align with the changes across South East London
