Home Health: Prepare clinical and progress notes. Maintain an accurate and complete record of observations, treatments, and care of patients. Provide education and counsel patients and families regarding care needs and related problems. Assist patients in learning appropriate self-care techniques. Performed required wound care and documentation of progression. Utilized aseptic technique, as appropriate. Perform medication reconciliation. Evaluate with registered nurse, the effectiveness of the LPNs nursing services. Performed documentation in Athena, Sandata, OASIS, and Brightree software.

Retrieved medication from Omnicell for pre-surgical dosing. Reviewed chart for necessary surgical form documentation. Reviewed patient input for preparation for discharge teaching. Prepared and restocked pre-op bays.
