Case Manager
Current- Assist clients who have experienced homelessness and/or economic hardship.- Create an individualized plan, through direct and collaborative assessments, to identify barriers that keep a client from finding long-term housing and help overcome those barriers.- Support clients who are living with physical or mental health diagnosis such as substance abuse, mental illness, traumatic brain injury, developmental disabilities, etc.- Collaborate with the on-site staff, property manager, and/or landlord as an advocate and intermediary between a client and staff member if lease violations or other housing complications occur.- Create eviction prevention plans with/for clients and assist them in meeting all goals of the plan.- Provide transportation and/or accompaniment for clients to housing-related appointments (eg, furniture procurement, lease signing, medical assessments, etc.). - Provide assistance with grocery shopping, cooking, budgeting skills, and other activities of daily living as needed to ensure success once in housing.- Understand and uphold the Vulnerable Adults Act, HIPPA guidelines, and other regulations.- Assist clients in locating/accessing community resources that relate to housing, food, evictions, expungements, etc.- Perform outreach to the community and other appropriate agencies regarding housing.
