Currently I am a case manager at Tyler Home Dialysis. I train patients on how to do their home dialysis as well as assist them in managing their fluid goals, diet and labs. I draw their labs and work with the doctors on managing their dialysis and labs for optimal clinical outcomes. Another important duty I perform is being on call via phone for the patients after hours. I call and check on patients on the weekends and am available weeknights to help them troubleshoot any issues with dialysis that they are having. I also do the ordering for the clinic and assist the patients with their monthly supply orders. Additional monthly duties include working with the monthly quality assessments and managing quality goals under CMS guidelines.

As a home care nurse I managed a caseload of patients who were at risk for rehospitalization due to exacerbations of chronic and acute illnesses. My goal was to allow them to safely manage their health concerns in the home. I achieved this through making home visits and phone calls to support and educate them on how to manage their illnesses. One of my most important duties was providing education on various topics such as diabetes, diabetic foot care, managing blood glucose levels, hypertension stage 3 and 4 kidney disease as well as providing nursing care in the home. Another of my duties was to assess patient's need for DME and supplies and order and maintain the supplies necessary for managing the patient's healthcare concerns in the home. I also was on call every other week for the patients and managed their health concerns over the phone as well as providing support and answering health questions for the patients and their families. I collaborated with patients and their families and physicians, physical therapists, occupational therapists, speech therapists, licensed social workers, and certified nurse assistants in order to help patients manage their health issues succesfully. Utilizing my assessment and communication skills I helped patients live safely at home with acute and chronic illnesses and after surgical events. My goal was to meet their needs and help them find solutions to increase their functionality and to decrease unnecessary hospitalizations.

As an RN case manager I was responsible for a caseload of 30 patients. I made visits to the home and phone calls in order to manage the patients' symptoms and to assist them in achieving their end of life goals. As a hospice nurse my primary goal was to provide my patients and their families with the specialized care and support they needed near the end of life. I provided the patients with the best possible care in regards to their individual concerns. I collaborated with the care team comprised of the patient,their families,physicians and DME companies to effectively manage the patients' symptoms. I ordered DME and collaborated with pharmacists as well to make sure the patient had the equipment and medications they needed to reach their end of life goals. We discussed problems as a team and educated those that were a part of the healing process for patients and families in order to provide quality healthcare. I also was the on call RN for over a year during my time working for hospice. I was available via phone for over 100 patients from Friday evenings until Monday mornings to help them troubleshoot any issues they were having, provide any support they needed via phone and anéw questions and concerns that they had. One of the other requirements during this time was to check on all new patients or patients with symptom management issues daily via the phone to help them manage their care at home and provide support.

I began my career at Fresenius as a PCT and continued to work as a staff nurse upon the completion of my associates of nursing degree. As a staff nurse it was my responsibility to perform safe, effective hemodialysis treatments and to support and educate my patients on their disease processes and to assist them in managing their chronic health issues. I also collaborated with other team members, including the social worker, renal dietitian, patient and doctor in managing their care.
